Tdap BOOSTER IMMUNIZATIONS FOR 6TH GRADERS (2011-2011 School Year)
TO: Parents of students who will enter the sixth grade in September 2011
• All immunizations must be up to date.
• Effective, July 1, 2006, a new State law requires all 6th grade students to have a booster to their diphtheria, pertussis, and tetanus (DPT) immunizations within the last five years.
• Obtain the Tdap booster vaccine documentation from your doctor, military clinics, Bon Secours Care-A-Van, the Richmond City Health District and/or appropriate medical facilities.
• Any sixth-grader, who has not complied with the state immunization law, will not be allowed to attend school until the student has met this requirement.
